STATEMENT OF AGREEMENT AND AUTHORIZATION
Please read the following statement thoroughly:
I certify that the information provided in the application for employment is true, correct, and complete to the best of my knowledge. If employed, any misstatement or omission of fact on this application may result in dismissal. I authorize the investigation of all statements and information contained in this application. I release from all liability anyone supplying such information and I release the employer from all liability that might result from making an investigation.
I understand that should an employment offer be extended to me and accepted that I will fully adhere to the policies, rules and regulations of employment at Aria. I understand that acceptance of any offer of employment does not create a contractual obligation upon the employer to continue to employ me in the future. Employment at Aria is "at will" for no specified duration and may be terminated by either Aria or myself at any time, with or without cause.
I understand that a reference check and/or background check will be conducted on me for employment purposes. By agreeing to this statement, I hereby authorize Aria to contact any and all corporations, former employers, educational institutions, law enforcement agencies, city, state, county, federal courts, and military services to release information about my background including but not limited to, information about employment, education, criminal record and general public history records to Aria.
I release from all liability all persons, companies, and schools supplying such information. I indemnify Aria against any liability, which may result from making such requests. The release shall remain in effect for the length of my employment. I understand that I may have a right to request additional disclosures regarding the nature and scope of the investigation.
